Mazak Quick Turn 10 Turrent Index Problem

My '84 Quick Turn 10 turret jams up on clamping a lot. Seems like the turret over rotates and then the cervic coupling is too misaligned to engage. It does this in both directions, which makes me think it isn't just simply misaligned.
Any advice on what I should dig into first?
